This notice explains how Subsidium Registry (“Subsidium,” “we,” “us,” or “our”) handles information submitted through subsidiumregistry.com and related communications.
Current status of the nurse network
Subsidium is developing a professional nurse network with a future focus on community healthcare facilities. Submitting the interest form is not an application for a specific opening or an offer of employment or contract work. It does not guarantee network acceptance, placement, shifts, facility access, or future contact.
Information we collect
When you submit the nurse-interest form or contact us, we may collect:
- Contact information, including your name, email address, and optional phone number.
- Professional-interest information, including a referral source if you provide one and an optional message about your experience or scheduling interests.
- Communications and information you provide in follow-up messages.
- Technical and website-use information, such as IP address, browser or device information, cookies, pages visited, and security or spam-detection data collected by our hosting and security providers.
Do not submit sensitive information
The current form does not accept file uploads. Do not submit Social Security numbers, tax or banking information, government identification, copies of licenses or credentials, medical or health information, patient names or records, protected health information, or other highly sensitive information in the message field or by ordinary email. If additional documents are needed in the future, we will provide separate instructions.
How we use information
We use information to review interest in the developing nurse network, communicate with you, understand professional background and referral context, plan future services, operate and secure the website, prevent abuse, maintain appropriate business records, and meet legal obligations.
How submissions are handled
The website is hosted by WordPress.com/Automattic. Information submitted through the nurse-interest form is routed to Subsidium’s dedicated business email account for review. Our website, email, security, and spam-prevention providers may process limited information as needed to provide their services.
Service providers and disclosures
We may disclose information to service providers that help us host, secure, and operate the website and email system; to professional advisers or contractors who need it for authorized work and are expected to protect it; or when required to comply with law, protect rights or safety, or investigate misuse. Current core providers include WordPress.com/Automattic for website hosting and Titan for business email.
Subsidium does not sell nurse-interest information or share it for cross-context behavioral advertising.
Retention
We retain information only for as long as reasonably necessary for the purposes described in this notice and as required or permitted by applicable law. Retention periods may vary by record type. We may delete information when it is no longer needed and deletion is legally permitted.
Your choices
You may ask to access, correct, or delete information you submitted, withdraw your expression of interest, or ask us to stop non-required communications. We may need to verify your identity and may retain information when required or permitted by law. Depending on where you live, applicable law may provide additional rights.
Cookies and external content
The site and its providers may use cookies and similar technologies that are necessary for security, site operation, and limited analytics. Content embedded from another website may allow that provider to collect information as if you visited that website directly. You can use browser controls to limit cookies, although parts of the site may not work as intended.
Security
We use reasonable administrative and technical safeguards appropriate to the current operation, but no website, email system, or storage method can be guaranteed completely secure.
Children
This site and nurse-interest process are not directed to children under 18, and we do not knowingly request information from them.
